Program Guidelines

Our Dental Care Club is designed to provide greater access to quality dental care at an affordable price. It is a discounted fee schedule for most dental services, only good at Greeley Dental Care. You save on everything from cleanings and fillings to crowns and cosmetic procedures.

GUIDELINE DETAILS‍

Patient portion of the bill is due the day of service.
There is a $50 reinstatement fee if your plan lapses.
Cannot be used in conjunction with other dental plans.
Care credit may not be used to pay Club premium.
Non-refundable and/or non-transferable.
Excludes complex dental work.

This program is a discount plan, not dental insurance. The Dental Care Club annual premium must be paid with either cash, check, or credit card.

Dental care club exclusions & limitations

For treatment, in the sole opinion of our doctors, lies outside their scope.
For referrals to specialists or specialized services.
For hospitalization or hospital charges of any kind.
For cost of dental care which are covered under automobile medical.
For services or injuries covered under workers’ compensation.
